으로 알려져 있고 얼굴 중앙 부이에 생기는 광선 과민성 피부 위축과 홍반이다. 환자의 절
반 가량이 신장 질환 증상 및 혈관 쇄약으로 인한 신경병증과 발작이 생길 수 있다. 치료는
코티코스테로이드를 만성적으로 투여한다.

Down's syndrome (다운 증후군) 21번 염색체의 전부나 일부를 침범하는 과도한 염색체 물
질에 의해 특징적인 안모, 지능저하 등이 나타나는 발육장애. 흔하게 나타나며 발생율은 임
신부의 나이가 많아짐에 따라 증가된다. 임상적 소견으로는 지능저하, 동그란 얼굴, 전두부
돌출과 후두부 평편화를 동반한 단두증 등이 특징이다.

이되지는 않는 근막 발통점.

malignant hypertension (악성 고혈압) 고혈압증의 하나의 병태를 나타낸 것이고 임상적으
로는 대개 확장기 고혈압(통상은 130mmHg)인데, 안저에는 유두 부종이 있으며, 급속하게
진행하는 심기능 장애를 수반하고, 방치하면 대개 요독증으로 사망하는 특징을 지닌다. 본태
성 고혈압에서 진행하는 것이 많고 신질환 등으로 잇달아 발생하는 수도 있다.

mechlorethamine (메클로레타민) 최초의 nitrogen mustard P 알킬화 항암제이다. 심한 국소
작용 때문에 메클로레타민은 임상적으로 정맥내로 투여하며, 투여후 체내에서 몇분 이내에
물 혹은 세포성분과 결합하여 불활성 화합물로 변한다. 따라서 특정 부위로 가는 혈액순환
을 일시적으로 차단하면 정상 조직의 손상을 막을 수 있으면 또한 암조직에 분포되는 동맥
내로 직접 주사하여 약물을 국소에 농축시킬 수 있다. 호지킨병(악성 육아종증)에 유효하며
다른 임파종에도 효과적이다. 그러나 제 3∼4기의 호지킨병에 대하여는 vincristine,
procarbazine, prednisone과 혼합하여 사용한다. 균상 식육종에 0.25% 희석 용액을 도포한다.
주된 독작용은 오심, 구토, 백혈구 감소증, 혈소판 감소증 등이며 가끔 피부발적 그리고 여
성은 월경불순, 기형아 출산 등을 초래할 수 있다. 근래엔 백혈병 및 기타 골수증식 질환에
의 사용 빈도가 줄어들고 있으며 기관지, 난소, 유방암 등의 치료도 다른 약물로 대치되고
있다.

pain disorder (통증 장애, 동통 장애) 동의어=psychogenic pain disorders. 통증을 심하게 호
소하는 임상 상태로서 적절한 신체 소견이 없으며, 통증의 발병, 심한 정도, 악화, 유지에 심
리적 요인이 역할을 한다는 증거가 있다.

retrospective clinical study (소급된 임상 연구)

S. aureus (황색 포도상 구균) 색소 산생, coagulase 양성, mannitol 발효성의 병원성 균종.
황색 포도상 구균은 운동성이 없는 구균으로 직경이 0.2에서 1.0마이크로미터이다. 불규칙한
포도송이 모양으로 혹은 짧은 사슬로 나타난다. 그람 양성균이나 배양후 오래된 세균이나
탐식된 경우에는 그람 음성으로 나타난다. 통성 혐기성이나 호기성 환경에서 더 잘 자라며
호흡과 발효를 통해 에너지를 얻는다. 병원성 결정 인자로 표면 항원(협막 항원과 단백 수
용체), 세포외 효소(혈장 응고 효소, 지질 분해 효소, 히아유로니다제, 스타필로키나제), 그리
고 독소(알파, 베타, 델타, 감마 독소, 류코시딘, 장 독소, 표피 박리 독소, 독소 쇽 증후군
유발 독소-1)을 가진다. 임상적으로 화농성 질환의 80% 이상의 원인균이다. 임상 증상은 피
부 감염(뾰루지와 옹종, 농가진, 표피 박리 증후군), 폐렴, 골수염, 균혈증과 심내막염, 전이
성 황색 포도 구균 감염증, 식중독을 일으킨다.

salivary gland carcinoma (타액선 암) 고급 악성도이며 이하선(80%)에 발생하며 60대, 남성
(80%)에 호발한다. 단단하고 무통성인 종물이다. 악성도가 심해서 보통 종양 발생후 일년
내에 임상가에게 발견된다.

salmonella (살모넬라) 장내균과의 그람 음성. 보통 혐기성 세균의 1속으로서 아포를 형성하
지 않는 간균이며 보통 주모성 편모에 의한 운동성을 갖는다. 구연산 염을 유일한 탄소원으
로 이용하며 일반적으로 포도당을 발효시키거나 자당 이나 유당은 발효시키지 못한다. 이
균속은 O, Vi 및 H 항원을 기초로 하는 균종 혹은 혈청형으로 분류한다. 균체 항원은 혈
청형 분류의 기초가 되며 A에서 E군까지의 균체 항원 및 Vi 항원에 대한 혼합 다가 항혈
청을 이용한 응집반응에 의하여 사람 및 하등동물에서 분리된 균주의 95%에서 혈청형을 검
출할 수 있다. 살모넬라의 H 항원은 이상성을 나타내어 특이 항원성을 가진 위상 1 혹은
비특이 항원성을 가진 위상 2로 표시한다. O, H 및 Vi 항원성의 분류에 따라 1500 이상의
혈청형이 확인되어 있다. O 항원의 종류는 숫자로 표시하며 위상 1의 H 항원은 소문자의
알파벳으로, 위상 2의 H 항원은 숫자로 표시한다. 임상 검사실에서는 흔히 3균종, 즉 S.
typhi, S. choleraesuis 및 S. enteritidis를 혈청학적 및 생화학적 반응을 기초로 분류하여 이
중의 하나로 보고한다. 앞의 두 균종을 제외한 모든 혈청형은 S. enteritidis에 속한다. 균종
명으로 잘 알려진 많은 균주들은 따라서 S. enteritidis의 혈청형으로 표시된다. 분류학적 목
적으로는 살모넬라가 생화학적 특성을 기초로 하여 5 아속으로 분류되며 항원성을 기초로
하여 균종으로 구분된다. 아속과 선택된 대표적인 혈청학적 변이주는 다음과 같다. 아속 I
(대부분의 혈청형을 포함) : S. choleraesuis, S. hirschfeldii, S. typhi, S. paratyphi-A, S.
chottmuelleri, S. typhimurium, S. enteritidis 및 S. gallinarum. 아속 II : S. salamae. 아속
III : S. ariyonae. 아속 IV : S. houtenae. 아속 V : S. bongor. 이 살모넬라속은 장열 (장티
푸스 및 파라티푸스), 패혈증, 위장염을 일으키는 병원성 균종을 포함하고 있다. 가장 흔한
임상증상은 식중독이다. 살모넬라균종은 하등동물에 널리 분포되어 있으며 빈번하게 질병을
일으킨다.

로서, 그 특징이 사고의 형태와 내용의 장애(유리연상, 망상 또는 환청), 기분의 장애(둔감,
둔마 또는 부적합한 감정), 자기자신에 대한 감각이나 외부 세계와의 관계의 자애(자아 한계
의 상실, 비현실적 사고, 자폐적 후퇴), 그리고 행동의 장애(기이하고, 목적이 없고, 판 에 박
힌듯한 활동 또는 비활동) 등이다. 과거에는 정신분열증의 개념에 대한 정의나 임상적 적용
등이 극히 다양하였다. DSM III-R의 기준이 강조하는 점은 현저한 사고의 장애(망상, 환청,
또는 감정이나 행동의 자앵에 동반되는 다른 사고장애), 과거의 기능수준의 퇴폐, 만성(6개
월 이상의 기간)인 점과 급성, 한계성, 단순성 또는 잠복성 정신 분열증이라고 하던 분류 조
건 등을 제외한 것이다. 원래는 dementia인격에 통합의 결여와 분리가 있다는 점을 강조하였다.

somatoform disorder (신체 장애, 신체형 장애, 신체화 장애) 기질적 소견이 없는 신체 질환
을 나타내는 증상이 있는 정신적 상태로서 임상적으로 적응성 기능에 심한 장애의 원인이
된다. DSM-VI는 이러한 장애에 5가지가 있다는 것을 알려준다. 즉 신체화장애
(somatization disorder), 전환장애 (conversion disorder), 통증장애 (pain disorder), 건강염려
증 (hypochondriasis), 그리고 신체이형성장애 (body dysmorphic disorder)이다.

T. pallidum (매독트레포네마) 사람의 성병성 매독과 비성병성 매독 모두의 병원체. 매독의
원인균이며 조밀한 규칙적인 나선 모양이며 1개내지 8개의 측원섬유를 가지는 혐기성 균이
다. 매독, 요스, 팬타 등의 임상질환을 일으킨다. 배양이 불가능하고 항원 구조에 대해서도
상세히 알려져 있지 않다.
